APSEA's Lifestyle Change Program

Let APSEA help you track your health and progress to reduce the risk for and/or manage your hypertension and diabetes, free of cost!

About our program

APSEA's Lifestyle Change program was designed to consistently follow up with patients that we find to have elevated blood pressure and blood glucose levels at our screenings. This program provides screening participants the opportunity to have regular check-ins with APSEA volunteers and healthcare professionals to consistently monitor blood pressure, blood glucose, and lifestyle habits to keep you on track. 

The lifestyle change program is completely free of cost, and you are free to join or leave at any time.

Our goal is to help you!

​Our program includes:

  • Monthly virtual sessions on preventive health topics (diet, physical activity etc.) for 6 months

  • Monthly at home* screenings which include: blood pressure, fasting blood sugar, A1c testing, body mass index, waist circumference, body fat, and heart rate.

    • *Participants can choose to have their monthly screening conducted at their home or at an APSEA screening location​

  • Weekly correspondence from an APSEA volunteer to ensure you are on track with your diet/exercise routine and to answer your health-related questions

  • One-on-one coaching sessions with a CDC lifestyle coach, trained APSEA volunteers, and Dr. Aayush Visaria, MD, MPH as needed.

The National Diabetes Prevention Program (National DPP) is a partnership of public and private organizations working to prevent or delay type 2 diabetes and hypertension. Partners make it easier for people at risk for type 2 diabetes to participate in evidence-based lifestyle change programs to reduce their risk of type 2 diabetes.
